Volunteer Waiver and Release of Liability

I, the undersigned volunteer, acknowledge and agree to the following:

1. Assumption of Risk
   I understand that volunteering at a running event involves potential risks, including but not limited to accidents, injuries, exposure to weather, interaction with participants and spectators, and other unforeseen hazards. I voluntarily assume all such risks, both known and unknown, associated with my participation as a volunteer in this event.

2. Release and Waiver
   I hereby release, waive, discharge, and hold harmless Cottonwood Heights Parks and Recreation, its officers, directors, employees, agents, sponsors, event organizers, and other volunteers (“Released Parties”) from any and all liability, claims, demands, losses, or damages arising out of or related to my participation as a volunteer in the 2025 Thanksgiving Day 5K, whether caused by negligence or otherwise.

3. Medical Treatment
   I authorize any necessary medical treatment in the event of injury or illness during the event and agree to be responsible for any resulting medical costs. I understand that medical support may be provided at the event, but there is no guarantee of immediate or on-site care.

4. Photo/Media Release
   I grant permission to the Cottonwood Heights Parks and Recreation and its representatives to take photographs, video, and other recordings of me during the event and to use them for promotional purposes in any format, including social media, website, print, and other publications, without compensation.

5. Code of Conduct
   I agree to conduct myself professionally and respectfully during the event, to follow the instructions of event organizers and staff, and to help maintain a safe and positive environment for all participants, staff, and volunteers.

6. Minors
   (If under 18 years of age): This waiver must be signed by a parent or legal guardian.
