Participant Waiver for Wurst Tri Ever Event
Event Details: Wurst Tri Ever event to be held on October 12, 2025
Release of Liability and Assumption of Risk
I, [Participant Name], hereby waive and release Personal Best Events, LLC, sponsors, partners, event affiliates, its chapters, directors, officers, administrators, representatives and executors, all city and state governments, and all related personnel (collectively referred to as "Releasees") from any and all claims, liabilities, or causes of action arising from my participation in the Wurst Tri Ever event on October 12, 20245 This release extends to past and present employees, volunteers, agents, representatives, and assigns.
Inherent and Potential Risks
I understand that participating in the Wurst Tri Ever event involves strenuous physical activities such as swimming, bicycling, and running long distances, which may span several hours. I acknowledge and assume all risks associated with these activities, including but not limited to collisions, falls, weather effects, and contact with other participants or volunteers.
Safety Measures and Requirements
I agree to dress appropriately for the event to mitigate risks, including wearing suitable shoes and complying with safety gear requirements, such as a helmet meeting CPSC standards, throughout cycling portions of the event.
Medical Considerations
I attest to being medically and physically fit to participate in the Wurst Tri Ever event. In case of any doubts regarding my ability to participate safely, I agree to seek medical advice and take responsibility for my decision to participate.
Emergency Medical Care
I consent to receiving emergency medical care and transportation as necessary by medical professionals in case of injury during the event. I also release the Releasees from any liability associated with such emergency medical services.
Voluntary Participation and Compliance
I acknowledge the risks involved in participating in the Wurst Tri Ever event and agree to abide by all event rules, safety procedures, and decisions of event officials to ensure a safe environment for all participants.
Photography Release
I grant permission to Personal Best Events, LLC to use any media recordings of my participation in the event for promotional and commercial purposes.
Parental/Guardian Consent (if applicable)
For participants under 18 years old, a parent or legal guardian must provide consent, assume risks, and agree to the terms outlined in this waiver.
