Volunteer Sign-up Form

I know that volunteering in a road race is a potentially hazardous activity and that I should not enter and participate unless I am medically able and properly trained. I assume all risks associated with participating in this event. Having read this waiver and knowing these facts and in consideration of accepting my entry, I and anyone entitled to act on behalf, waive, release, and hold harmless the Ashland Cherryland Healthy Community Collaborative, Jason Beck, Office of Supervisor Nate Miley, the County of Alameda, all and any of its departments, divisions, agencies, agents, officials, officers, employees, and any other persons associated with the County, all race volunteers and all sponsors, their representatives, employees and successors from any claims and liabilities of any kind arising out of my participation in this event or carelessness of the persons named in this waiver. I also understand that entry fees are non-refundable and entries are non-transferable. 

A parent/guardian shall sign for any person under the age of 18.

I accept that I have read all the terms and conditions and I agree to the risks and the release of liability.
